How to Obtain a “Certificate of Attendance” For a Driving Permit
The State of Georgia Department of Driver Services requires that minors be in enrolled in school, have no more than 9 days of unexcused absences, and have not been found in violation of offenses as stated on the “Certificate of Attendance” form.
There are three ways to obtain a “Certificate of Attendance” form:
1 . The following Department of Driver Services link:
2. From the Reception area at the front of DECA
3. From the Registrar’s Office (See forms outside office door)
Scholars should fill out the top portion of the form ONLY. Next, they should turn it in to Mrs. Gonney (Registrar) or Mrs. Shepherd (Receptionist) to complete processing.
The rest of the form needs to be completed by a school official and notarized at the school. There is a box to submit forms outside the Registar’s Office, if either person is not available.
Pickup for completed form will be Fridays. Scholars would then take it to the Department Of Driver Services. Please note forms may not be available if they are submitted the day before pick up.
