The State of Georgia Department of Driver Services requires that minors be in enrolled in school, have no more than 9 days of unexcused absences, and have not been found in violation of offenses as stated on the “Certificate of Attendance” form.

• A notarized Certificate of School Enrollment Form found Here

• Transcript provided by the customer's most recent school of attendance. The transcript does not have to be certified. Transcripts can be printed online in most school districts. 
• Most recent progress or grade report.  
• A current school identification card, issued by the school or school system.
• Other forms of school identification.

To obtain a “Certificate of Attendance” form:

•       Complete the DDS Enrollment Form: Certificate of School Enrollment Form 2023 (DDS-1).pdf 

•       Submit it to the Reception area at the front of DECA to be completed by a school official and notarized.

• Pickup for the completed form will be every Fridays at 1pm. Scholars would then take it to the Department Of Driver Services.  Please note forms may not be available if they are submitted the day before pick up.
